If you prefer to lodge your complaint on line, please complete the following on-line complaints form. If you have any questions about this form, or need help completing it, please contact us on (08) 9323 0600 or country free call 1800 813 583. Once completed, your complaint form will be assessed and we will contact you. If you do not hear from us within 5 working days of lodging your complaint on-line please contact us. If your complaint is within jurisdiction, we may need to obtain signatures from you to allow us to access further information from the provider. Should your complaint not be within our jurisdiction you will be contacted by our office, and where possible we will refer you to other organisations who may be able to help. As we cannot accept anonymous complaints, please ensure your contact details are correct prior to submitting your complaint.

We require that all questions are answered, as this will allow us to effectively deal with your complaint. You will not be able to submit your complaint unless all mandatory fields are completed.
