For your convenience you are able to lodge a complaint with our office on-line or you may download a complaint form and return this to our office by hand or post. Written complaints may also be submitted by fax, email and through the post. Please go to the Contact Us page for details. Remember you have the right to complain. Just making a complaint may help to improve the standard of care for you and others. There are penalties under the Act to prevent a service provider from penalising a person for making a complaint to this office. Contact us for more information if you are concerned about this.

Often the best way to resolve a complaint is to directly approach the service provider with your concerns (the legislation requires a consumer to take reasonable steps in resolving the matter with the provider). Our staff will usually encourage complainants to try to resolve the complaint with the service provider prior to lodging the complaint with our office. However, in some circumstances this may not be possible or a favourable outcome cannot be reached by the parties involved. It is a good idea to keep records of all communication between the provider and complainant and copies should be sent to us with the complaint form. Our legislation requires that all complaints be in writing. We will also need to obtain written authorisation from complainants in order to pursue the complaint. This authorisation allows us to approach providers and to discuss the concerns raised with them, and to access relevant information from records relating to the complaint. Should you choose to lodge your complaint via the internet (on-line) and your complaint is within our jurisdiction, an authorisation form along with a reply paid envelope will be sent to you for your signature.
