If you are not satisfied with the outcome or processes followed by our office, in resolving the complaint, you may, preferably in writing, request an internal review on your file or you may complain to the State Ombudsman directly.

Internal Review

This involves an internal review of the file by a senior officer who, wherever possible, has had no prior involvement in the matter.

The objective of the review is to consider:

  • the issues raised by the complainant or provider in their request for an internal review;
  • the adequacy of the original enquiries undertaken and whether all relevant information was gathered and appropriately considered;
  • whether reasonable conclusions have been reached and whether any other conclusions should be considered;
  • whether sensible and workable outcomes have been achieved; and
  • whether any further additional work should be undertaken on the matter.

Public Interest Disclosure – Office of the Public Sector Standards Commissioner

The Public Interest Disclosure Act 2003 (the PID Act) provides an avenue for people to raise concerns about matters of public interest in the State public sector without fear of reprisal.

The PID Act was established to encourage openness and accountability in government by providing protection to people, whether they are an employee of a government organisation or a member of the public, who raise concerns about improper or unlawful activities in public authorities.

If you wish to make a disclosure about improper or unlawful conduct you should firstly seek advice from a Public Interest Disclosure Officer. Corruption and Crime Commission

One of the roles the Corruption and Crime Commission is to deal with allegations of corrupt misconduct by public officers.

The Commission accepts and can investigate allegations of misconduct by Western Australian police officers and public officers including officers employed in local government.

'Misconduct' occurs if a public officer corruptly acts or corruptly fails to act in the performance of the functions of office or employment. Misconduct also occurs when a public officer corruptly takes advantage of their office or employment to obtain a benefit for themselves or others, or, whilst acting in official capacity commits an offence punishable by two or more years of imprisonment.
